Vision Home Health Care LLC was founded on a simple but powerful belief: every individual, regardless of disability, deserves to live with dignity, independence, and a genuine sense of belonging in their community. Our story began in 1999, when our founder, Dr. Uchenna Sabina Okeke-Nnamah, worked as a caregiver at the West Side Mental Institution in Chicago, Illinois. Witnessing the daily limitations and hardships faced by individuals with developmental and intellectual disabilities moved her deeply. She made a personal commitment to do something meaningful to improve their quality of life. That promise eventually led to the formal establishment of Vision Home Health Care LLC on October 29, 2008. Today, we proudly serve adults ages 18 to 65 across Davidson, Rutherford, and Sumner counties in Middle Tennessee. Our clients are developmentally disabled and may present mild to moderate behavioral challenges. We also provide DCS Supported Living and Foster Home services for children ages 0 to 18. If a determination is made that services are no longer appropriate — whether by our team, the individual, or their guardians — we continue providing care until another provider can be located.
