From our DCS Foster Home and Supported Living programs for children ages 0–18 to our adult services like Residential Habilitation, Nursing, and Respite, we're here for every stage of life. Our team is fully trained, background-checked, and committed to your safety. We provide 24-hour supervision when needed, but we never forget that independence and dignity matter just as much. We're certified with DCS, DDA, and MCOs; and we stand by a promise we never break: we will never abandon a client during transitions. CHOICES
The CHOICES program in Tennessee is a TennCare program that provides long-term care services and support for seniors and adults with physical disabilities. It helps individuals live independently in their homes or communities rather than in nursing facilities.
1915 C
1915(c) refers to a set of Medicaid waiver programs that provide Home and Community-Based Services (HCBS) for people with disabilities. These waivers allow states to use federal Medicaid funds to support individuals in their homes and communities rather than requiring them to live in institutions.

Respite I Supportive Home Care I Transportation
Respite care is planned or emergency temporary care provided to caregivers of an adult. Respite programs provide planned, short-term, and time-limited to run errands, and breaks for families. Caregivers can be rewarding and often strengthens bonds within a family. At the same time, caregiving can be demanding amidst life's other responsibilities. Family caregivers and those in their care can benefit from a little time off now and then.
Supported Living and Foster Homes
We provide safe, supervised community-based homes and nurturing foster family placements for children with developmental delays or mild to moderate behavioral challenges, in partnership with the Tennessee Department of Children's Services
Transportation Services
Individual Transportation Services: The agency will provide non-emergency transport of consumers as they travel to and from doctor’s appointments and other community activities. The provision for securing transportation will be implemented in the most safe and cost effective manner to facilitate the transportation needs of those individuals being served.
